Health Forms

Students in the categories below must provide physical examination forms dated and signed by a physician within one year, along with all required immunization records. Please check with your child's doctor's office for the date of their last physical exam. The following grades must provide this information by the first day of school:

• All new students in all grades
• All 9th grade students
• All 7th grade students
• All Kindergarten students
• All Montessori students

In addition, all action plans (food allergy, bee sting allergy, asthma, diabetes, seizures, etc.) must be completed, dated and signed by a physician every year. See more information here.

Uniforms

Uniforms too small, worn out, or do you just need more? Now is a great time to replace or add to your student's uniform wardrobe for next year. See school uniform information here. Many items are sold in The Peddler School Store. Peddler hours are listed here.
