School Year Forms & Information
FOR EACH STUDENT:
- View the division tabs for health information, athletics and registration.
- View the grade-level sections for information specific to certain grades.
If your child is new to Visitation, please contact his or her previous school to have the school release your child's previous year's final grades to us. In most cases, our application files contain only partial grades from the most recent school year; we do need the previous year's finalized grades in order to maintain and ensure the continuity of your child's permanent school file. Please mail all final grades to the Enrollment Office at Visitation.
Division & Grade-Specific Information

Health, Athletics & Contact Information
Visitation registration for the upcoming school year is now open! We use this system to collect health, emergency and athletic information for the 2022-23 school year.
An email has been sent to one parent per family in order to prevent multiple registrations per student. If you are not the parent who is likely to complete the form for your child(ren), please forward it and, if you choose, let us know so that we can note this in our records.
Athletic Registration
- Athletic registration will take place later in summer prior to fall sports.
Health Forms
- Upper School-level student-athletes need a current sports qualifying physical (current within three years).
- TCO offers free sports physicals. See details here.
- New students and students (entering grades M Pre-K, K, 7 and 9) need a new physical on file with School Nurse, JoAnne Geiser, as well as an Action Plan for any severe allergies or health conditions before they are eligible to participate. See the Health Office page for more information regarding specific needs for physicals, allergies and other health conditions.
- Health clearance from the School Nurse is required in order to be eligible for any athletic participation

ImPACT Concussion Screening
- Visitation's Twin Cities Orthopedic Certified Athletic Trainer, Emily Tindal, will send each student-athlete who needs to complete an ImPACT Concussions Screening an email during the first week in August with her individual code to complete the screening at home. An ImPACT screening does not prevent a concussion, so student-athletes will not be withheld from opening practices if it is not yet completed. However, a completed screening is mandatory to compete in the first competition.
- Screenings are required for the following high-impact sports:
- Soccer
- Swim/Dive
- Volleyball
- Alpine
- Basketball
- Hockey
- Lacrosse
- Softball
- T&F Pole Vault.
- Screening is available for student-athletes in remaining sports upon request.
