School Year Forms & Information

FOR EACH STUDENT:

  1. View the division tabs for health information, athletics and registration.
  2. View the grade-level sections for information specific to certain grades.

If your child is new to Visitation, please contact his or her previous school to have the school release your child's previous year's final grades to us. In most cases, our application files contain only partial grades from the most recent school year; we do need the previous year's finalized grades in order to maintain and ensure the continuity of your child's permanent school file. Please mail all final grades to the Enrollment Office at Visitation.

Please do not hesitate to contact the division director should any questions arise. The division directors will be at school during June and August.

Upper School Director - Anna Barter

Please consider donating disinfecting wipes (Clorox, Lysol or any generic/store brand) and unscented hand sanitizer for community use in classrooms and shared spaces. Thank you.

Health, Athletics & Contact Information

Visitation registration for the upcoming school year is now open! We use this system to collect health, emergency and athletic information for the 2021-22 school year.

An email has been sent to one parent per family in order to prevent multiple registrations per student. If you are not the parent who is likely to complete the form for your child(ren), please forward it and, if you choose, let us know so that we can note this in our records.

How do I get started?

Check your email for a specific snapcode link that will take you to your child's registration form.

What if I have more than one student at Visitation?

You will receive a separate email for each student containing a unique snapcode or snapcode link. Please only open one link at a time to prevent technical difficulties. Once you complete one form, you may go back to your email to begin the next form(s) for any additional child(ren).

Should I create an account?

The snapcode link will take you to the PowerSchool Registration site. You will use the same login information you used to complete registration last year. Or, if you are new to Visitation, you will use the same login information you used to complete your online application. You should use the same account to complete forms for multiple children.

CONCUSSION SCREENING

All student-athletes planning to participate in Alpine Skiing, Basketball, Diving, Hockey, Lacrosse, Soccer or Softball at the high school level are required to have a current ImPACT concussion screen on file in the athletics office prior to their first competition. Athletes may not participate without a current screen. All screens are valid for two years from the test date.
